- If you decide that you’d like to give by consistent, Electronic Funds Transfer, it’s easy to begin:
- • Print and complete this Authorization Agreement;
- • Attach a “voided” check from your checking or savings account and mail it,
along with your authorization to the address below;
- • The automatic deduction will begin according to the date you request in
your authorization;
- • You’ll be sent a receipt in the mail each time your donation is made;
- • Your authorization will remain in effect until you decide to stop it.
